Coyote Creek Golf Course – 222 Clubhouse Drive, Fort Lupton, CO 80621
- Format is a 4-player scramble. Each player tees off and the best shot is chosen. All players then hit again from within one club length of the chosen shot. You cannot move closer to the hole, or out of a hazard or the rough. NO SCORE WORSE THAN BOGEY CAN BE TAKEN ON ANY HOLE.
- Prizes awarded for team winners (1st, 2nd, and 3rd), special holes (Long Drive, Closest to the Pin, etc.) and a drawing to be held after the tournament.
- ENTRY FEES ARE $110 PER PLAYER AND ARE DUE WITH THE COMPLETED ENTRY FORMS NO LATER THAN SEPTEMBER 19, 2018.
Any player/players added after this date or the day of tournament (after exceeding 100 players) will be required to pay an additional $20.00/ player late fee. The shotgun start is scheduled for 8 am, with the registration beginning at 7 am.

Dress code policy: For men collared shirts are required, no cutoffs or other athletic shorts. Ladies must have either collared or sleeved shirts. No Tank Tops allowed for anyone.
Mulligans will be sold with the proceeds going to Triangle Cross Ranch which provides assistance to intellectually disabled adults.
ENTRY INCLUDES: GREEN AND CART FEES, RANGE BALLS, GIFT BAG, PRIZE MONIES, BREAKFAST, AND LUNCH.
